Exotic Animal Appointment Request Form

The form below will ask for your personal information and your pet's medical history. This information will remain confidential and will not be shared, unless we have verbal/written authorized consent from you.

Only routine care and exams will be approved. Urgent or emergency care visits will be referred to TLC Animal Hospital to be treated by Dr. Vicky Dashley.

We will not be seeing birds, snakes, wildlife, or livestock animals at this time. Animals that can be treated at Stellar include turtles, tortoises, lizards, guinea pigs, rabbits, chinchillas, and ferrets. If your pet's species is not listed above but does not fall under the category of pets we will not see, please give us a call.

Once you have submitted your form, a Client Service Representative will contact you within 24 hours. Please keep in mind that we are closed for major holidays, and a response can be delayed for up to 48 hours from the initial form submission. Client Service Representatives will need to speak to you directly via phone call to set the appointment. They will collect additional information about your pet. Communication via text or email is prohibited for Exotic Pet appointments.
